A Great Start to a Lasting Relationship

Your pet has their own personality, routines, and medical needs, and we want to hear all about them! Your first veterinary appointment gives us a chance to learn what makes your pet unique while establishing a plan for their continued care.

During your visit, our veterinary team may:

  • Review your pet's medical history and previous veterinary records.
  • Perform a thorough physical examination.
  • Discuss nutrition, behavior, and daily routines.
  • Evaluate vaccination and parasite prevention needs.
  • Address any health concerns or changes you've noticed.
  • Recommend preventive care or additional testing based on your pet's needs.

Client Consent and Authorization

I authorize Bloomsburg Veterinary Hospital to examine, treat, prescribe medications for, and perform necessary medical or surgical procedures on my pet(s). I understand that the veterinary team will take reasonable precautions to protect my pet(s) from injury, escape, or other harm. However, I acknowledge that veterinary care involves certain risks and agree that Bloomsburg Veterinary Hospital will not be held responsible for circumstances beyond its reasonable control.

If my pet(s) remain at the hospital without arrangements for pickup, I understand that written notice may be sent to my address on file. If I fail to reclaim my pet(s) within ten (10) days of receiving notice, they may be considered abandoned and handled in accordance with applicable Pennsylvania laws. I understand that I remain responsible for all veterinary services provided, along with any applicable collection costs.

Photography and Media Consent

I grant Bloomsburg Veterinary Hospital permission to photograph or record videos of my pet(s) and/or me for use on the hospital's website, social media accounts, and other promotional materials. I authorize the hospital to identify my pet(s) by name and, when appropriate, include my first and/or last name. I release Bloomsburg Veterinary Hospital from claims related to the authorized use of these photographs or recordings.
